Age band

Ill or toxic appearance / lethargy

Hypotension, tachycardia out of proportion, or poor perfusion

Peritoneal signs (rigidity, rebound, guarding)

Bilious (green) vomiting

Bloody stool or 'currant jelly' stool

Scrotal/testicular pain or swelling

Pregnancy possible with abdominal pain or vaginal bleeding

Significant trauma or suspected non-accidental trauma

Infant < 3 months with persistent pain/vomiting

Altered mental status

Fever with focal abdominal tenderness

Persistent vomiting > 24 h or unable to tolerate PO

Signs of moderate–severe dehydration

Localized RLQ pain or migration of pain

Known sickle cell disease with new abdominal pain

Known DM or polyuria/polydipsia (DKA concern)

Immunocompromised or recent abdominal surgery

Severe pain > 6 h or worsening despite analgesia

Routine evaluation

No red flags selected. Routine outpatient evaluation appropriate if well-appearing. Provide return precautions.
