- ABCs, protect airway (consider intubation if GCS < 8 with concern)
- Dextrose if hypoglycemic; thiamine 100 mg IV before glucose if malnourished
- Naloxone if opioid suspected
- Empiric antibiotics + acyclovir if CNS infection suspected
